Ortho-K for Children: How Overnight Lenses Slow Myopia Progression

May 2024 Dr. Brandon Harnos 5 min read

Parents seeking a way to slow their child's worsening nearsightedness increasingly hear about Ortho-K. Here's why we recommend it — and what to expect from the fitting process at our New Paltz practice.

The Stakes of Childhood Myopia Progression

A child who becomes myopic at age 7 has many more years of growth ahead than one who starts at 14. Every diopter avoided through control treatment means a lower final prescription — and meaningfully lower lifetime risk of retinal detachment, maculopathy, and glaucoma. The difference between -3.00 and -8.00 in adulthood is often made during the childhood years.

How Ortho-K Controls Myopia

Ortho-K creates peripheral defocus — light from the visual periphery focuses in front of the retina rather than behind it. Research indicates this peripheral defocus signal inhibits the retinal response that triggers axial elongation. Multiple large clinical studies show approximately 60% reduction in axial growth compared to conventional glasses.

Candidacy and the Fitting Process

Ortho-K works best for children with mild to moderate myopia (up to approximately -5.00 to -6.00 diopters) and low astigmatism. Age is less a limiting factor than maturity — we've successfully fitted children as young as 7 when child and parents are committed to the routine.

We begin with corneal topography mapping, then select and dispense custom lenses. Follow-up at one day, one week, and one month monitors the corneal response. Most children achieve comfortable daytime vision within 1–2 weeks.

What Parents Should Know

Children adapt to lens insertion and removal more readily than parents expect. We spend significant time at the dispensing visit ensuring both child and parent are comfortable before leaving the office. We recommend having a backup pair of glasses available for any days without lens access.
