Keratoconus and Scleral Lenses in the Hudson Valley

March 2024 Dr. Scott Morrison 6 min read

Keratoconus can feel alarming at diagnosis — but with modern specialty lens fitting, most patients maintain excellent functional vision without surgery. Expert care is available right here in the Hudson Valley.

What Is Keratoconus?

Keratoconus is a progressive condition in which the cornea gradually thins and bulges into a cone shape, distorting vision in ways that glasses and soft lenses cannot adequately correct. It typically first appears in the teens or early 20s and may progress for 10–20 years before stabilizing.

Why Scleral Lenses Are the Gold Standard

Scleral lenses vault completely over the irregular corneal surface and rest on the sclera. The space fills with saline, creating a smooth, regular optical surface regardless of the cornea's shape. For most keratoconus patients, sclerals achieve visual acuity close to or equal to what a normal eye sees — a remarkable outcome given the degree of underlying distortion.

Our Approach at Harnos Optometry

We map your cornea with topography and OCT imaging to characterize the cone's location, steepness, and size. We then work systematically through diagnostic lens sets to find the optimal vault and landing characteristics. Keratoconus often requires multiple fitting visits — particularly in asymmetric cases where each eye presents differently.

We carry 30+ scleral lens designs and work with multiple specialty laboratories to accommodate the full spectrum of keratoconus severity. We also monitor for progression and coordinate with corneal ophthalmologists when collagen cross-linking (CXL) is indicated to halt progression.

Insurance Coverage

Keratoconus is a medical diagnosis. Scleral lenses for keratoconus are typically classified as medically necessary and partially covered by medical insurance. We help patients document medical necessity and navigate their coverage.
