What Happens at a Comprehensive Eye Exam at Harnos Optometry in New Paltz

Many patients arrive at their first comprehensive eye exam uncertain about what to expect. Here's exactly what we do at Harnos Optometry — and why each step is clinically important.

History and Intake

Every exam begins with a conversation about your current vision, any symptoms, your medical history, medications, and family history of eye disease. This history guides the clinical examination — a patient with family history of glaucoma gets a more intensive optic nerve evaluation; a patient with diabetes gets focused attention on their retina.

Preliminary Testing

Before you see the doctor, we gather objective measurements: visual acuity (with and without current correction), automated refraction estimate, intraocular pressure, and pupil testing. These establish baselines and flag areas requiring closer attention in the exam.

Refraction

The classic "which is better — one or two?" process uses a phoropter to find the lens combination that provides your sharpest, most comfortable vision at distance and near. This is both an art and a science — a skilled refractionist extracts significantly more information than an automated machine alone.

Slit-Lamp Examination

A high-magnification microscope allows detailed examination of the eyelids, cornea, anterior chamber, iris, and crystalline lens. Early cataracts, corneal conditions, inflammation, and dozens of other findings become visible here that aren't detectable with the naked eye.

Dilated Fundus Examination

The most medically important component. Dilation drops widen the pupil to allow examination of the retina, optic nerve, macula, and blood vessels. This is where we detect diabetic retinopathy, glaucoma, macular degeneration, retinal tears, and other conditions with serious long-term consequences. Vision will be mildly blurred and light-sensitive for 2-4 hours after dilation.
